Application/Renewal for a Certificate of Identity If you wish to receive the application form for Certificate of Identity, please send us a self-addressed stamped return envelope (size A4). Please indicate outside the envelope the type of form and the quantity required. To process your request, you have to submit the following documents together with your application form to the High Commission:- Completed application form Form E(6) and Form 6 Two passport size photographs Copy of valid travel document/passport Singapore Certificate of Identity (if applicable) Documentary proof from your college or other institution of study or from your employer certifying the necessity for your remaining stay in overseas Documentary proof from your spouse’s college or other institution of study or from your spouse’s employer certifying the necessity for your spouse’s remaining stay in overseas Photocopy of your Exit Permit/Notice of Exemption (if applicable) Original and a photocopy of your marriage certificate * Original and a photocopy of you and your spouse’s Identity Card(s) * An additional photograph is required upon the issuance of the Certificate of Identity. Any other relevant documentary evidence in connection with your application will expedite the processing by the Singapore relevant authorities * The original copies of your marriage certificate and Identity cards are required because these documents need to be certified by the High commission. In this regard, please forward A$2.00 per document as certification fee plus an additional A$2.00 for an official cover letter. Please pay in Australian Money Order (including A$2 for notorial cover letter) made payable to the "Singapore High Commission, Canberra". The fee for the issuance or extension of your Certificate of Identity will be collected only upon approval by the Immigration & Checkpoints Authority. (Fees are subjected to change without notice)
