Withdrawal of CPF contributions If you wish to withdraw your Central Provident Fund (CPF) contributions, please contact the following authority for the application form:- Central Provident Fund Board (CPF Board) 79 Robinson Road CPF Building Singapore 068897 Tel: (65) 6220-2422 Fax: (65) 6225-8732 http://www.cpf.gov.sg If your CPF withdrawal form requires the High Commission’s attestation, please call us to schedule for an appointment. Please bring the following when you appear at our office at the appointed time:- Your old and current passports (original copy); and One other original official photo-ID (indicating your signature) Please note that you need to sign the CPF withdrawal form according to the signature indicated on your photo-ID in the presence of our Consular Officer. We wish to inform you that due to legal implications, we are unable to certify your withdrawal form without you appearing in person at our office. Alternatively, you may choose to obtain the attestation through your local Notary Public. Upon completion of the form, you will have to forward your completed CPF withdrawal form directly to the CPF Board.
